KETAMINE THERAPY

Veteran Eligibility Requirements.
Must be a U.S. Military Veteran with any discharge status other than dishonorable.​
Must be a Florida resident.
Ketamine Treatment Program Process
Phase One: Intake Submission
The veteran must complete the initial intake form. The BLF Program Director will review the submission and respond within 48 hours.

Phase Two: Application and Referral
The BLF program director will contact the veteran and provide a secure link to the application via email. Once the veteran is approved and meets all program requirements, the Program Manager will refer them to BLF’s Ketamine treatment partner.
Phase Three: Initial Treatment Setup
The BLF Program Director will coordinate the veteran’s first treatment session. The veteran is responsible for completing all necessary paperwork and scheduling any remaining appointments for the full course of treatment.
Phase Four: Completition and Ongoing Care
Upon completing the treatment, the veteran must notify the BLF Program Director and submit any final required paperwork. Maintenance treatments may be available at the veteran’s expense.
